To comply with applicable federal law, state statutes, and University policy in maintaining the highest standards for the handling and use of recombinant DNA (with or without etiologic agents) and according to the UCI Campus Policy for Use of rDNA and Biological Materials, all service orders placed should have a copy of the appropriate "biosafety approval form" attached, if relavant, for the DNA samples to be sequenced.
All UCI users who have an approved "APPLICATION TO USE RECOMBINANT DNA (rDNA), ETIOLOGIC AGENTS AND rDNA WITH ETIOLOGIC AGENTS" should attach a photocopy of the form.

Your biosafety form must adhere to the following guidelines:
